Personal Computer Repair – Terms and Conditions

This form authorizes the UW-Stevens Point Information Technology Service Desk Staff to perform repair services on your computer. A valid photo ID is required at drop-off and pick-up of a personal computer. 

Clicking the box below indicates that you agree to the following:

  • You have read and understand the "Personal Computer Repair Services Agreement" and associated fees.
  • The Service Desk reserves the right to deny repair services to any device, at any time, for any reason.
  • You understand the UWSP Service Desk staff is trained to perform minimal computer and software service, and the Service Desk is not an authorized service provider.  
  • You understand the procedures to be performed are an attempt towards the repair of your computer which could result in loss of part, or all, of the data stored thereon, and the Service Desk makes no guarantee as to the success of its attempts.  
  • You understand the Service Desk reserves the right to remove or change your local computer password(s).  
  • You understand the Service Desk is not responsible for any peripheral/accessory loss that may occur while your computer is physically located at the Service Desk.  
  • You understand the Service Desk highly recommends backing up your data to an alternate location prior to dropping off a computer for service.  
  • You understand the Service Desk reserves the right to remove applications to aid in the prevention of unwanted threats to your computer. This includes but is not limited to Peer-to-Peer file sharing applications and games. The Service Desk may also make attempts to ascertain the cause of the original problem in order to help our customers in the future.  

Computer Reformat/Upgrade Appended Agreement Form

  • The Service Desk is not responsible for any data loss that may occur during the repair process, and customers are fully responsible for backing up their own files beforehand. Even if data recovery is requested and successful, some files may still be inaccessible due to damage or media failure.

Hardware Repair & Replacement

  • If replacement hardware is required to complete the fix of the device, the Service Desk will not purchase the hardware for you. 
  • The Service Desk does not have the tools for all types of hardware repairs.

Picking Up Repaired Devices 

  • You will be notified via email or phone call when your device is ready. 
  • This ticket being marked as CLOSED or RESOLVED indicates that the device has been returned back to the customer.

Potential Fees

  • Failure to Pick Up a Device
    • After 3 contact attempts, a $5 per day holding fee will be applied until the repaired device is picked up. 
  • Off Campus Residence
    • Students who live off campus will be charged $60 fee if their device repair takes more than 3 hours. 
    • Repair service fees are included in student housing fees for students residing in the Stevens Point residence halls.
