Description of Service
This service allows you to submit a general UWSP IT support request. Please click "Enter IT Support Request" (even if you do not have a UWSP account), or consider viewing our Service Catalog.
Requested Information:
All starred (*) items are REQUIRED for submission.
- Short Description of Issue (*): A short description of your request.
- Description (*): A detailed description of your request.
- Include any relevant error messages or steps to reproduce the problem.
- Attachment: Any files that you would like to include.
- Example: Screenshot(s) of an error message.
- Location of Issue (Building + Room): Location of issue, if relevant.
- Computer or Device Name(s): The assigned names of related UWSP computer equipment, if relevant.
UWSP Submitter Specific:
- Requester (*): Who is requesting this service?
- Usually this will be your contact information.
- Department (*): The department related to this request.
- Usually this will be the department you belong to.
- Additional Contacts: Add additional UWSP individual(s) as a contact on this ticket.
- Notify Contact(s): Check this box to send a notification to the above additional contacts, if relevant.
Public Submitter Specific:
- Your name (*): Your first and last name.
- Your email address (*): The email address UWSP IT will use to contact you.
- Your phone number: The phone number UWSP IT will use to contact you.
- Verification Code (*): Complete a challenge before you submit your request. This prevents abuse of our support system.
Request Guidelines
Please provide all required information and all known optional information during submission. The more information you provide, the quicker your request can be processed.
When submitting a request, please ensure that all information provided is accurate. UWSP IT staff may reach out to request further information or ask for clarification. During that time, your request will be placed on hold until you respond.

Procedure:

- Fill out and submit the online form.
- The UWSP IT team will review your support request.
- Further information may be requested if necessary. Please check your email inbox regularly for messages related to your request.
