• This form is for SOC interim's to remediate accounts. Filling out this from will let Service Desk know to wipe MFA methods and default password.
• To fill out this form, click Request Service
  • In the form, make the requester/username whose account needs to be remedied.
  • Feel free to edit the description if more information is needed for the user. 
  • Click Submit.
