Requesting a PointCard or Other ID

 

Overview

Students, Faculty, and Staff:

UWSP students, faculty, and staff can request new cards after they become students, when they are hired, or when they need a replacement PointCard.   

 

Other Card Types:

In addition to standard UWSP PointCards, the IT Service Desk also prints several specialty cards for campus partners and affiliated groups. These cards can be requested using the same form.

  • MSTC
    • For Mid-State Technical College students living in UWSP residence halls.  
  • Name Tag
    • Printed name tags for specific campus jobs.
  • Police Services
    • Official ID badges for UWSP Police Services personnel.
  • Volunteer
    • For approved campus volunteers who require identification.
  • Dorm Checkout Card
    • For Housing and Residence Life to issue checkout cards.
  • DSC Voucher Cards
    • Voucher cards issued through Dining and Summer Conferences

 

Replacement Card Situations

Card Stopped Working

Typically, a UWSP ID will not stop working due to normal use. If you do experience problems with your ID, and there are no visible signs of damage, there will be no charge for a replacement. You will need to bring in the original, non-functioning, card with you when you pick up the replacement card to have the replacement fee waived.  

 

Card was Lost or Stolen

When an ID is lost or stolen, it is critical that all services associated with that PointCard are deactivated as soon as possible. This prevents unauthorized access to your residence hall, your UWSP services, and UWSP buildings while using your name. See the Lost PointCard page for instructions on how to deactivate all services associated with a lost PointCard. A replacement for a lost PointCard will cost $20. The fee will be applied to a student’s bill or charged to an employee’s department at the end of the semester. The fee cannot be refunded if the original card is found after the replacement card is printed. When a PointCard is stolen, there is no fee for a replacement card, but proof of a police investigation into the theft is needed to waive replacement fees. 

 

Employee Status Change

If faculty and staff change positions at UWSP, they may require an updated PointCard.  There is no charge to get an updated PointCards due to changes in employment status at UWSP. 

 

Name Change

PointCards are printed using names on record with the Registrar's office. PointCards are printed with a student's preferred name on the front of the card, and their legal name on the back of the card. If a preferred name is not on record with the Registrar's office, the PointCard is printed with your legal name on both sides.  

To make a name change, go to the Registrar's Academic Forms page. Once you have submitted your requested name change to the Registrar's office, wait at least two business days before requesting a new PointCard. 

When filling out the form, please include your preferred name as it should appear on the card. That helps the IT Service Desk staff make sure the correct name is on the card when it is printed. 

When picking up the replacement PointCard, bring the original PointCard so it can be exchanged for the new card. 

 

Need Voter ID

Most student IDs are voter compliant. Voter compliant IDs have a signature line, an issued date and a voter ID expiration date, and your legal name all on the back of the card. Even after the expiration date, your card can be used as long as you also provide a document  that shows you are enrolled in the school at the time of the election. This can be a document such as an enrollment verification letter, class schedule, tuition fee receipt, or certified campus housing list.  

If your card is not voter compliant, you will not be charged for a replacement. 

 

New Card for Use with Door Locks

Due to changes in technology or procedures, occasionally UWSP will need to replace your existing PointCard. In these circumstances, there is no charge for a replacement card. However, you will need to bring in your old card when picking up the replacement card. 

 

Other

If your replacement request does not fall under any of the categories listed above, select Other as your reason on the form and briefly describe the situation in the description box. This option is intended for unique circumstances that do not match the standard replacement reasons. 

 

Picking Up Card 

ID will be printed as soon as possible after the IT Service Desk receives the request. Once the card has been printed, the work order ticket will be updated, and a notice will be sent to the person requesting the card saying the card is ready for pick up. Please don’t try to pick up the card before the notice has been sent. The card will not be ready. 

If you're picking up a replacement card, please bring your old PointCard to the IT Service Desk to exchange it for your new card. If your card was lost or stolen, bring a valid form of photo ID. If your card is stolen, please bring information about the police investigation. 

 

Student Voter ID Information

For students, if you sign the back of the PointCard, it can be used as a Voter ID. We recommend using a fine tip felt pen for signing the PointCard. Be sure to give the ink time to dry! There is an expiration date on the back of the card. If the card is expired, you must also provide a document – paper or electronic - that shows you are enrolled in the school at the time of the election. This can be a document such as an enrollment verification letter, class schedule, tuition fee receipt, or certified campus housing list.  All other PointCard functions will continue to operate after that date. 

 

 

Procedure:

  1. Select the "Requesting a PointCard or Other ID” button.
  2. Fill out and submit the online form.
  3. Further information may be requested if necessary. Please check your email inbox regularly for messages related to your request.
  4. The IT Service Desk will print the card and send a notification when the card is ready to be picked up.
  5. After pickup, fees will be applied in certain circumstances.
