How to prepare and submit a campus project request.

This article provides detailed information on how to complete fields within the Request a University Project form.

Fields

Title or Name for this Request

Please choose a unique, descriptive name for the request. Generally, 2-6 words.

E.g., "Bio 100 Teaching Lab Microscope Replacements" or "Convert CBB 526 to Meeting Room"

Description and Justification

Please provide a brief description of objectives and justification for the request. This can include opportunities, deficiencies, or maintenance. Please include a description of any space renovations or IT requirements.

Requested Priority

Select the desired priority for this request. 

  • Mandatory: The requested work is required to meet legal or or accreditation requirements, UW-System mandates or regulations, or DFD projects.
  • Must do:
  • Should do:
  • Would like to do:

Consistency between departments is not expected and will be balanced with other areas as requests are ranked.

Please note that this is the college or department's opportunity to communicate the ranking of requests within their area. Please use aim to use the full spectrum of "Must do, Should do, Would like to do" when possible. If all requests are submitted with the same priority, little information on the department or college's internal priorities to the review committee.

Request Year

Select the calendar year that work on the request is desired to begin in.

Division

Select the division the request originates from.

College or Dept

Enter the college or department the request originates from. Examples are CPS, COFAC, Facilities, or IT.

Primary Contact

Select the primary contact for the request. This individual will be contacted for any questions on the request and notified of the final decision.

Space

If modifications will be made to a space, please list the building and rooms which will be modified.

Desired Completion Date

Enter the desired completion date for this request. If approved, this date will be used as a guide for scheduling but actual completion date will depend on required work and resource availability.

Estimated Cost

Enter the best available estimated cost of the request. Because this is early in the request process, ball-park estimates are acceptable at this stage. Estimates may be refined by IT or Facilities as part of the review process or developed in a latter planning stage prior to execution.

If quotes or priors are available, please use those and include supporting documentation in the Attachments section.

If quotes are not available, please us the Project Estimating Calculator (available in the Attachments section in the right menu of this article) where possible to estimate costs for common needs and include this document in the Attachments section of the request form.

Funding Sources

List each funding source, whether the funding is secured or proposed, and amount from each funding source. Funds total should equal estimated cost. E.g.:

  • FY24 Department funds, secured $20,000 and
  • FY24 Lab Mod, proposed, $15,000

-or-

  • Funding not yet identified, $35,000

External Implications

Are there external project implications that will potentially cause this project to have a higher priority ranking?
