Surveys Microsoft-Forms Forms

UWSP offers the following cloud-based survey tools to students and employees.

Qualtrics offers advanced logic and a powerful feature set to help you design and distribute complex surveys.
Microsoft Forms is an easy to use survey tool available from your UWSP Office 365 web portal. Microsoft Forms is perfect for lightweight surveys and overall data collection while still offering important features like branching and file upload. Microsoft Forms also integrates closely with Microsoft's Power Automate which can use incoming form responses to automate workflows where needed.

► IMPORTANT: When designing and distributing surveys, UWSP employees must not share or request highly sensitive data.

Highly sensitive data includes:

Restricted (High Risk) information which includes FERPA-protected information such as student grades, schedules.
Proprietary UWSP data.

Sign In To UWSP's Survey Tools

Go to uwsp.qualtrics.com.
If you are prompted with a sign-in screen from UWSP, sign in with your UWSP account and MFA. If you are working on a UWSP computer you may be signed into Qualtrics automatically.

The First Time You Access Qualtrics

If you are new to Qualtrics you may need to click the button, "No, I don't have a preexisting account here" to complete your Qualtrics sign in.

Once signed in, you can create and edit your own surveys and any surveys shared with you.

Sign in to your UWSP Office 365 Web Portal with your UWSP account and MFA. If you are working on a UWSP computer you may be signed in automatically.
On the left menu click Apps.
At the right, click All apps —>
Select Forms.
